Medical Records

Dickinson County Healthcare System maintains a medical record for each patient treated at our hospital.  To request a copy of your medical records, please contact our Health Information Management office between the hours of 7:00 am to 5:00 pm Monday through Friday at (906) 776-5473.

Prior to receiving your records, you will be asked to complete a medical records release authorization form and to provide proof of your identification with a valid government issued photo ID.  For your convenience, a copy of the medical records release form is provided on this page for download.  If you are requesting a records transfer for continuing care purposes, the form can be filled out and faxed to (906) 776-5416.

Records needed for continuing care purposes will be sent directly to your provider.
