Our case management team provides case coordination, discharge planning and family support with teams of specially trained registered nurses and social workers to help you with your discharge. With direction from your doctor, the case management team may see you to discuss post hospitalization services and equipment.

Scheduling Outpatient Services
After you leave, you may need to return for outpatient follow-up visits. Sometimes these appointments are made for you before you leave the hospital. If not, you may call our Central Scheduling Department at (906) 776-5383 or (888) 482-2345 to schedule hospital follow-up services.
Once you have been scheduled for hospital services, we can create your record for this visit if you call us in advance. This will allow us to serve you better on the day of your visit. It will be particularly helpful if you are a first-time patient or any of your demographic information has changed. When calling, please have your insurance information available.
