LPN - Full Time - Primary Care


Essential Responsibilities:

  • Utilizes the nursing process as the basis for nursing activities and care rendered.  Demonstrates  compliance with applicable standards as well as clinic a d hospital wide policies and procedures 
  • Maintains and utilizes consistently appropriate, professional and effective communication skills with providers, physicians, patients , staff, business representatives, and other healthcare agencies while providing excellent customer service for all encounters
  • Documents patient care activities in EHR or paper chart
  • Completes non-patient care duties in a timely manner
  • Maintains professionalism in the community, protects patient confidentiality and service as a patient advocate at all times. Maintains professional growth
  • Observed age-specific competency for all age groups
  • Ability to float to any other OCHS clinics as requested by manager and performs duties as assigned


Works under the direction of hospital employed physicians and mid-level providers (Nurse Practioners or Physician Assistant) to provide quality patient care in the medical office setting. Participates in the planning, implementation and evaluation of patient care within the objectives, standards,  and policies of the department and within the parameters of his or her preparation.

Minimum Qualifications:

Current Ml license as a practical nurse; Current WI license as a practical nurse, or ability to obtain Wisconsin license within 3 month of hire;  BCLS certification with AED instruction

Previous clinic, office and /or outpatient nursing

Interacts appropriately and positively with peers, patients, families, physicians and professionals


Demonstrates clinical competence; Venipuncture, when needed, after proper training; Basic knowledge of Immunization schedules from birth through adulthood; Ability to work cooperatively & communicate effectively; Perform other related duties as assigned
